If you notated the damage on your delivery ticket, please fill out the form below. If you found damage after the ticket was signed please read the information in the blue box for instructions.
When reporting the damage to the delivery freight terminal, they will supply you with a concealed damage claim form.
Once you submit the form to the delivery company and they validate your claim, they will issue a check directly to you for repairing or replacing the damaged item.
Unfortunately, as indicated in your purchase contract we are unable legally to file a damage claim on your behalf unless the damage is written on the delivery ticket (before the ticket is signed at the time of delivery).
